Fresh Start Survey

The Fresh Start Survey is a two-step process in which you will indicate your interest in up to 5 WVU Student Organizations. Once you have completed the survey, your information will be delivered to the president of each organization that you select, and you will then be contacted by a representative of each organization with further information and details about their respective organization.

In step one of the survey, you will select up to 5 organizations from those listed by clicking on the check-box next to the name of each org. You can also review a brief description of any organization by clicking on the org. name in the list. Once you have made your selections, please click on the button at the bottom of the page labelled "Submit and Continue" to continue to step two of the survey.

In step two of the survey, you will fill out a short form to provide your contact information and related details which will be stored with your survey response along with the orgs you select in step one. This information will only be used by WVU Student Organization Services and the representatives of each org. in conjunction with this survey. Your contact information will not be shared with any other party.
