Course Requirements

  1. A minimum of 300 clinical hours each semester (30% of your grade or 30 points)

  2. Monthly injury summaries (15% of your grade or 15 points)
    1. For each month summarize the injuries that occurred sport by sport
    2. Discuss if there is a pattern of injuries or any unique injuries, if the Dx was easy to obtain, and what would you do different with the rehabilitation protocol used.
    3. A minimum of one page single-spaced and computer generated for each monthly summary.  Attach a cover page with name, class, injury summary# and due date.
    4. Each monthly injury summary will be turned in on the dates specified in the due date outline by 12:00 p.m.  You will be expected to turn in summaries for January, February, March and April.
  3. BODY OF CASE REPORT FORMAT (20% of your grade or 20 points – 15 for the written + 5 points for the oral presentation)

Case reports will be from your clinical assignments.  All case reports will be computer generated and include the following information: use the following as headers.

a.       PERSONAL DATA - provide the age, gender, sport/occupation of the individual

b.       CHIEF COMPLAINT

c.       HISTORY OF PRESENT COMPLAINT – including signs and symptoms

d.       RESULTS OF PHYSICAL EXAMINATION – example – “Physical findings relevant to the rehabilitation program were…”

e.       MEDICAL HISTORY – surgery, laboratory results, examination, etc

f.         DIAGNOSIS – what was the injury or ailment

g.       TREATMENT AND CLINICAL COURSE – rehabilitation until and after return to competition

h.       CRITERIA FOR RETURN TO COMPETITION

i.         DEVIATION FROM EXPECTATIONS - a brief description of what makes this case unique.  What does the literature say about this injury?  (use journals not textbooks).

j.         REFERENCES - (separate page) after the case report and place the references used in the order they appear in the paper.  Your references should be in JAT format.  Refer to Authors Guide (J Athl Train.  2005;40(4):381-382).  Include 5 peer reviewed journal references.  Please do not include Phys Sportsmed as one of your references.

k.       Release of information form – no header but should be after references.

GUIDELINES

Refer to the following case reports and use as an example when writing your own.  Note that you are asked to give more specific information in your case report because the introduction and discussion is eliminated.  This case report should be computer generated with a cover page and page numbers.

Eberman LE, Cleary MA.  Celiac disease in an elite female collegiate volleyball athlete: a case report.  J Athl Train.  2005;40(4):360-364.  Leone JE, Gray KA, Massie JE, Rossi JM.  Celiac disease symptoms in a female collegiate tennis player: a case report.  J Athl Train.  2005;40(4):365-369.

A. Because of the format in the course outline for ATTR 623, only refer to the body of the case report.

1. You do not need to have an introduction or discussion in the case report.  Make sure that you are very detailed and specific in your writing.  This information should come from your SOAP notes you have already completed.

2. The case report should include the following components in order as listed in the course outline: personal data, chief complaint, history of present complaint (including symptoms), results of physical examination, medical history (surgery, lab results etc), diagnosis, treatment and clinical course (rehabilitation program until and after return to competition with sets, reps, weights and ROM – this part is a summary not as detailed a description used in ATTR 625), criteria for return to competition, and deviation from expectations.

3. It is in the deviation from expectations where you include information from your five references.  (Note: the five references could also be used in the other sections).  Your case report will be due the day you do the PowerPoint presentation.

B. The oral presentation (PPP) will be 20 minutes in length - 15 minutes for the case report including all the pertinent information listed above and 5 minutes for discussion, which will include 3 questions to generate discussion with the class.  Dress professionally.

  1. Any educational concerns or issues during the semester should be directed to me, as well as your supervising certified athletic trainer if applicable.

Two points will be taken off each monthly injury summary, while five points will be taken off the body of the case report assignment when they are turned in late.  Those points will be taken off after the assignment has been graded.

If you do not turn in an assignment due to an absence, you must notify the instructor either prior to the class that you will be unable to attend or that same day if you are ill.  In the event of extenuating circumstances, this syllabus may be altered or changed by the instructor.
